How to Enter a Team and Entry Form
1. Download and print out an OFFICIAL MBNC ENTRY FORM.
ALL INFORMATION MUST BE READABLE OR YOUR ENTRY WILL BE RETURNED.
2. Complete the basic team and contact information.
3. Select a JERSEY OPTION (see page 2 of entry form).
Option 1 – Provide your own Jersey
A refundable Jersey Deposit of $200 is required with your entry fee.
The minimum standard is a light and dark jersey with legal basketball numbers on both the front and back. A reversible jersey is acceptable. Shorts of the same color are also strongly requested.
Option 2 - Buy the “Standard” MBNC Jersey
You will receive 12 MBNC reversible jersey priced at $260. These are a high quality 100% Poly-Dazzle jerseys with contrasting trim on a v-neck and armhole with individually hemmed bottoms and are available in both a men's and women's version. This option will have the MBNC logo on the front and legal Basketball numbers front and back. The color is white with a reversible red. Men's sizes are XS-3XL and Women's sizes are XS – 2XL. These jerseys will meet your needs for several years or can be used as a souvenir item for you players.
Option 3 - Buy the MBNC jersey “Customized” to your specifications.
You will receive 12 reversible jerseys priced at $325. These are the same jerseys described in Option 2 except they will be imprinted with the art work of your choice along with legal basketball numbers front and back. The available colors are: Black,Forest, Maroon, Navy, Purple, Red and Royal. You may view the jerseys at the following websites:

ALL JERSEY ORDERS ARE PLACED THROUGH THE MBNC HOME OFFICE AND PAYMENT MUST ACCOMPANY YOUR ENTRY FEE.
Art work files may be e-mailed to MBNC and will be forwarded to the supplier.
4. Complete the Team Roster Information
We require ALL player information on the entry form. This includes height, age, birth year, school attended or home town and pro experience, if applicable. We print a top quality souvenir program and it is important that personal information is accurate and complete. YOUR ENTRY WILL BE RETURNED IF THIS INFORMATION IS NOT COMPLETE.
5. Write a Check for the Entry Fee and Jersey option chosen.
Entry fee prior to entry deadline: $700.00
Entry fee after entry deadline (3/29/08) $800.00
(assumes space available in age division requested)
If you have selected Jersey Option 1, (bring your own), please write a separate check for $200. We will not deposit this check. It will be returned to you at your first game upon inspection of your jerseys.
If you have selected Jersey Option 2 or 3, you should include the cost of the jerseys in your entry fee check.
6. Entry Notification
MBNC takes entries on a first come, first entered basis. Each correctly completed entry form accompanied by the correct fees will be date stamped and numbered by age division upon arrival at the MBNC home office.
Entries arriving on the same day are arranged in order of the postmark or shipping date.
MBNC staff will notify you by e-mail or phone of your team entry number immediately after receipt of your acceptable entry form and fee.
Checks are payable to “MBNC, Inc.”
No team will be considered entered until the full entry fee and the appropriate jersey fee has been received.
THERE ARE ABSOLUTLEY NO EXCEPTIONS TO THIS POLICY!
